Confirmation of participation in the Shefayim Water Park -

On 18.8.21 on Wednesday there will be an exit activity to the Shefayim water park. The activity is for the general public and shuttles will depart from various centers around the country.

My son / daughter hereby confirms to take part in the activity that will take place in the Shefayim water park on 18.8.21 on Wednesday. Departure hours - at 7:30 from Be'er Sheva and at 8:30 from Tirat Carmel with a stop and pick-up from Or Akiva.

I know that the departure is made by shuttle from the cities of Beer Sheva, and Tirat Carmel. Depending on hours to be arranged in advance. Going to the activity is the responsibility of the parent.

I measured the temperature for my child and found that the body temperature was below 38 degrees Celsius.

My child does not cough / cough and has no breathing difficulties (except for a cough or difficulty breathing due to a chronic condition such as asthma or other allergies).
